Nurse Practitioner Hospice-Non Profit-Atlanta, GA

Reporting to the COO of the organization, the Nurse Practitioner (NP) will provide hospice and palliative care services to patients in various settings but not limited to nursing home, rehabilitation, assisted living, and hospice. This faith based organization truly lives up to their strong mission and delivers the “best of the best” quality of care.

Qualifications:

  1. Current Adult Nurse Practitioner (ANP) or Geriatric Nurse Practitioner (GNP) certification required
  1. Minimum 2-5 years NP experience in hospice, palliative care, and/or geriatrics
  2. Current unrestricted Georgia NP license
  3. DEA/Prescriptive Authority; prescriptive privileges
  4. Strong clinical skills in advanced illnesses and determining terminal status in patients
  5. Valid GA driver’s license, without restrictions
  6. Ability to work independently
  7. Strong time management and organization skills
  8. Strong communication skills and interpersonal skills
  9. Conflict resolution skills; ability to deal tactfully/patiently with patients/families/staff
  10. Ability to communicate effectively both verbally and in writing
  11. Maintain effective working relationships with multiple related communities/groups

 

Responsibilities:

  1. Provide hospice and palliative care to a group of patients in various settings; nursing home, rehabilitation, assisted living, clinic, hospice
  2. Work with the hospice team, facility staff, Primary Care Physician (PCP), other NP and licensed practical nurse (LPN)
  3. Perform face-to-face visits per hospice regulations in a timely fashion as needed
  4. Assist hospice RNs with symptom management, medication/treatment recommendations as needed
  5. Collaborate with PCP/facility staff/patients/families to individualize plan of care
  6. Obtain health histories
  7. Assess, diagnose, develop plan of care, treat and evaluate
  8. Prescribe medications
  9. Manage health problems & coordinate care
  10. Refer to specialists as necessary
  11. Provide Advance Care Planning
  12. Document in the Electronic Medical Record (EMR): maintain timely record completion
